Should I bring a list of my medications to my office visit?

Please supply us with your pharmacy address and phone number as well. Bringing your medication and supplements to your doctor’s visit takes a little more effort, however, it will ensure that you are prescribed and taking the right medications.

Do you accept insurance?

We welcome Medicare and all major PPO insurance plans. We also welcome the following managed care plans: Lakewood IPA, Applecare, CareMore, Talbert, HealthCare Partners, Prime Health Care, Access, Primary Care Associates, Pioneer, Brookshire IPA and Los Alamitos IPA.

What is your “short notice cancellation” fee?

Your appointment time has been specifically reserved for you. We ask that you call a day in advance to reschedule or cancel your appointment. A $25 missed appointment fee will be charged to your account if you do not notify us within 24 hours. Please note that procedures, surgeries and vasectomy cancellations will incur additional costs.

How do I get a hold of my doctor after hours?

In an emergency or life-threatening situation, please call 911 or seek immediate medical attention at your local ER facility. In the matter of urgent matters, call our office and ask to speak with the doctor on call.

Can I request medication refills?

For medication refill requests, the pharmacy will contact us electronically or by fax or phone. We strive to respond to your request within 48 to 72 hours. If your insurance requires a prior authorization, that will take additional time. Medications are not refilled over the weekends and holidays.

Charge for other forms?

A charge of $25 is required for release of medical records. Other forms, including but not limited to insurance, state disability and travel will be charged at $5 per page.
